The town of Templeton Board of Health will be holding an H1N1 Flu Clinic on Saturday, December 19, 2009 at the Narragansett Regional Middle School from 9:00 am until 2:00 pm. This clinic will be for children six months to eighteen years of age, NO ADULTS WILL BE VACCINATED AT THIS CLINIC.
Consent forms are available at www.templeton1.org, www.nrsd.org, and www.phillipston-ma.gov. Please fill out the consent form and bring this form with you on the day of the clinic.
You must contact the Templeton Board of Health @ 978-939-2377 between the hours of 8:30 am and 3:30 pm, Monday – Friday, to reserve a vaccine for your child.”
