I, _____________________________, hereby agree to the conditions set forth by the Highway Superintendent of Phillipston and agree to complete all the necessary work to comply with these conditions prior to my receipt of a driveway entrance permit. I understand that no occupancy permit will be issued until the driveway has been completed.
*** If A Scenic Rd, You Must See Planning Board ***
(Templeton Rd, Burntshirt Rd, Searles Hill Rd, Baldwin Hill Rd, Common-Baldwinville Rd To Athol Rd, Lincoln Rd, Williamsville Rd, Narrow LN.) ANY stone wall openings must have finished end points and must consist of - tapered ends turning back towards the driveway, stone piers, granite post. Or wooden post.
Conditions:
❑ Camber & swail to conform and follow water run off to ditch.
❑ Minimum of 20’ by 12’ corrugated steel or plastic culvert pipe.
Head walls on each end of pipe to be cement.
❑ Compacted 6” or 12” crushed gravel over pipe.
❑ Paved apron 3” of asphalt. Paved from roadway to property line of owner with 6” gravel base. ❑ Crushed gravel apron 6” thick from roadway to property line.
❑ Landing approach before ditch line.
❑ Before paving, contact Superintendent.
❑ Driveway not to be relocated from plans.
❑ If a Scenic road stone wall must maintain finished

❑ Driveway staked out.
❑ Apron lines painted for inspection prior to paving.
❑ Front edge of Mailbox must be minimum of 18 inches from the edge of the road 41/2’ Radius ❑ Culvert maintenance is the landowner’s responsibility. _______________________

A plan of proposed work must accompany this application.
Driveway permit fee: $50.00 – No re-inspection fee.
Make checks payable to the Town of Phillipston – Permit expires 1 year after date of issuance.
If an extension is needed the applicant must apply in writing to the Superintendent. If granted, the extension will be for 3 months.
